Notes:
Gaige's crosstable, based on that of the American Chess Bulletin of Oct. 1912 (p.220), shows scores for Blake against Michelsen and Lee against Michelsen as '0?' for both players, and for Blake against Hokensen and Blake against Lee as '1?' for both players. Di Felice resolves these as wins for Blake against Michelsen, Lee against Michelsen, Hokensen against Blake, and Blake against Lee. The crosstable in the Saint Louis Globe-Democrat of 1 Sep. 1912 (p.40) agrees with Di Felice in these four instances, but shows six of Palmer's games as forfeits, none of which are indicated by the American Chess Bulletin, Gaige or Di Felice. These are Palmer's games against Elliott, Michelsen, Blake, Widmeyer, Lee, and Hine. The forfeits against Michelsen and Widmeyer are only indicated by the 0 scores in Lee's row of the crosstable, but not by the 1's against Lee in his column of the crosstable. The others are indicated in both places, but I assume that there were, in fact, six forfeits. The crosstable in the Saint Louis Globe-Democrat has other inconsistencies, however. It shows Hokensen-Hine as a draw for Hokensen but a loss for Hine (I assume it should be a draw). It shows Hokensen-Widmeyer as a draw for Hokensen but a win for Widmeyer. The American Chess Bulletin, Gaige and Di Felice show this as a loss for Widmeyer, which I have assumed is correct.
